Salary:-$12.10/hour to $15.49/hr depending on experience
  

Job Responsibilities:
  • Job initial located in Franklin, VA with a transition to have a home base location of Suffolk, VA
  • Will consider applicants for a Senior Service Coordinator position with relevant work experience and required credentials.

JOB SUMMARY

 

This position is responsible for the coordination of client services.

 

MAJOR DUTIES

 

  • Receives and processes referrals; contacts parents/guardians to discuss needs and services and to provide information.

     

  • Participates in the determination of eligibility.

     

  • Participates in the development of Individual Family Service Plans.

     

  • Participates in transition planning.

     

  • Ensures compliance with confidentiality requirements.

     

  • Refers clients to appropriate service providers.

     

  • Attends required training.

     

  • Completes progress notes, charts and required reports.

     

  • Investigates and resolves problems related to services provided to families.

     

  • Performs related duties.

Job Qualifications:

 

  • Knowledge of Early Intervention Program policies and procedures.

     

  • Knowledge of disabilities services principles.

     

  • Knowledge of needs assessment strategies.

     

  • Knowledge of infant and toddler development.

     

  • Knowledge of agency policies and procedures.

     

  • Skill in the operation of computers and job related software programs.

     

  • Skill in decision making and problem solving.

     

  • Skill in interpersonal relations and in dealing with the public.

 

  • Skill in oral and written communication
